Athletic Clearance
All students interested in joining sports at Maywood must complete the following steps:
1. Sports Physical- Obtain a copy from the athletic director or coach and return
the completed document to the athletic director or your coach. The form must be
fully complete with signatures and a stamp for nurse approval.
A copy can be downloaded and printed from https://www.lausd.org/Page/15807
2. Paperwork on athletic clearance website- On your computer go to
https://www.athleticclearance.com/ and log in or create an account. Once
registered, start the process by clicking “START CLEARANCE HERE” and
complete all sections with your parents.
3. Proof of Insurance- Upload a copy of your insurance card to the athletic
clearance website. If you do not have insurance, contact the athletic director for
assistance or purchase school insurance through K and K insurance

4. Code of Conduct- All students interested in participating in athletics must
adhere to the athletic code of conduct outlined on the athletic clearance website.
Students must have good academic standing, demonstrate good sportsmanship
on and off the field, and adhere to all school rules and policies.
